Supporting People - Support Plans
Everyone who receives housing support (under Supporting People arrangements) should have a support plan. This is a document that describes the assistance that will be given to an individual person, who will provide it and how often. It should be reviewed with the individual on a regular basis to ensure that their needs are being met.
